This is a Riverside Transport sponsored program, offering employees the option to purchase cellular service at the corporate monthly rate plan that Riverside receives from its wireless provider. Selected lines of service included may be new phone numbers or an existing line of service may be moved from any carrier into the program. Riverside offers this program through a management portal jointly provided by Hyperion Partners, LLC and MobilSense Technologies Inc.

SERVICE OPTIONS

A T-Mobile rate plan including unlimited talk, text and data is offered at a monthly flat rate of $22.00 including taxes and fees. A one-time set up fee of $20 will occur in the initial month of service. The discounted monthly rate plan is not guaranteed to be the lowest market price.

PAYMENT TERMS

This service is post-paid, meaning invoicing occurs after a month’s usage cycle is completed. Employees are solely responsible for all monthly cellular bill charges associated with the assigned phone number. Payments are due monthly. The credit card provided by the employee at the time of enrollment will be charged monthly until service is cancelled, transferred out, or otherwise removed from the program by the employee, employer, or provider.

Failure of a payment transaction for any reason will result in suspension of phone service on day 7 after the payment is due. Cancellation of the line will occur on day 14 after the payment is due. It is the responsibility of the employee to provide a new payment method as soon as possible to avoid interruption or disconnection of service and to update credit card information when a card is expiring or expired.

DEVICE OPTIONS

Electronic credit card payment may be used to purchase from a catalog of selected reconditioned phones at a substantial discount through the managed website. Employees may also bring their own device if it is eligible to be transferred to or used on the T-Mobile network. It is the Employee’s responsibility to select a suitable device for this program. Reconditioned phones are limited to a 30-Day warranty replacement policy.

TERM OF SERVICE

There is no required time limit for service to remain active and employees may enroll at any time. Service may be terminated at any time and for any reason. You will be charged a pro-rated fee based on the number of days with active service in your final billing cycle. Hyperion reserves the right to refuse service to any participant in the program

FACTORS AFFECTING SERVICE

Your T-Mobile actual Service area, network availability, coverage and quality may vary based on a number of factors, including your selected service, network capacity, terrain, weather, if you are on a private or public Wi-Fi network, using a non-T-Mobile device, or if your Device no longer supports network technologies compatible with or available on T-Mobile’s network. Outages and interruptions in Service may occur, and speed of Service varies. Devices also have varying speed capabilities and may connect to different networks depending on technology. Even within coverage areas and with broadband-capable devices, network changes, traffic volume, outages, technical limitations, signal strength, obstructions, weather, public safety needs, and other conditions may impact speeds and service availability.

WHAT HAPPENS IF MY DEVICE IS LOST OR STOLEN?

If your device is lost or stolen, immediately contact the Help Desk so that the service may be suspended until the service can be transferred to a new mobile handset.
